Print and bring them, or ask at reception and we'll give you a clipboard — either is fine.
New patient forms
- Patient registration — your contact details, who to call in an emergency, and how you'd like us to reach you about appointments.
- Medical and dental history — conditions, medications, allergies and past dental work. This matters more than people expect: some medications affect healing, bleeding and anaesthetic.
- Insurance and billing information — your plan details so we can bill the insurer directly instead of you paying up front and claiming it back.
Other forms
- Consent for treatment — signed before any procedure, after we've gone through what's involved.
- Release of dental records — if you're transferring from another dentist, or moving on from us.
What to bring with them
- A piece of photo ID
- Your insurance card, or your Canadian Dental Care Plan member number
- A list of medications, including over-the-counter and supplements
- Any X-rays or records from a previous dentist, if you have them
